Leopards News · Physicals Must Be Completed Before Participation


Physical information is on the athletic site. You can get the information by clicking “more” on top of the page and you will see mandatory paperwork. All students wanting to play a sport must have completed physicals and proof of insurance to be able to participate. All students must start an account and fill out the information www.athleticclearance.com. Please make sure you click on Florida and the 2019-20 school year.  There will be information that needs to be filled out with your medical history and parent/guardian information.  Your physical with the Dr. signature, date of the physical and a picture of your insurance card must be uploaded on to this sight. Parents and students have an section they have to electronically sign as well to be eligible to participate.  This process will take about 15 minutes to complete.  If you play more than one sport, make sure you check all the sports the student might be interested in trying out/participating in.  If a student is on a team, they must pay the county mandated $50 participation fee. If they play a second sport, they must pay an additional $30 dollars per county policy.  They max student-athletes will have to pay is $80 total.

Also, all student-athletes will have to watch videos on concussions, heat illness and sudden cardiac arrest on www.nfhslearn.com/courses. This is mandated by the state (FHSAA).
